1. (a) The department shall furnish application forms for 20 day permits at the division of motor vehicles, 23 Hazen Drive, Concord, N.H. 03305 and at any substation.

2. (b) The application for a 20 day permit shall include the following:

   1. (1) Name, address, date of birth, and driver license number;
   2. (2) Proof of identity, as described in Saf-C 517.03;

   3. (3) Description of the vehicle, including:

      1. a. Make, model, color;
      2. b. Vehicle identification number; and
      3. c. Year of manufacture;
   4. (4) Name and legal address of person selling the vehicle;
   5. (5) Proof of ownership;
   6. (6) Certification by the applicant that they are a resident of New Hampshire, and that they have not previously applied for or been issued a 20 day permit for the vehicle;
   7. (7) A statement that the vehicle meets all New Hampshire inspection requirements and is in safe operating condition;
   8. (8) A statement that all the information on the application is true; and
   9. (9) Signature of the owner.
3. (c) An application for a 20 day permit shall be submitted to one of the locations described in (a) above.
