1. (a) The department shall notify the certificate holder by first class mail, at least 30 days prior to the expiration of a certificate the following:

   1. (1) The date that the certificate expires; and
   2. (2) The requirements for the renewal of the certificate, pursuant to these rules.
2. (b) All renewal notices shall be sent to the certificate holder’s last known address.
