- (a) A carrier may request, in writing, to the bureau, that its certificate or permit be suspended.
(b) The written request shall contain:
- (1) The reason for the request;
- (2) The effective date of the suspension; and
- (3) The duration of the suspension.
- (c) The duration of the suspension requested in (b) above shall not exceed one year.
- (d) No later than 30 days after the end of the suspension period, the carrier shall request the bureau, in writing, to either reinstate or revoke the authority.
