1. (a) Each applicant for an oil discharge and pollution control license shall complete and submit form RT-50, “Application for Oil Discharge and Pollution Control License.”

2. (b) Each applicant shall furnish the following on form RT-50:

   1. (1) Applicant’s company name, mailing address and telephone number;
   2. (2) Applicant's federal employee identification number “FEIN”;

   3. (3) Type of business entity, as follows;

      1. a. Corporation;
      2. b. LLC;
      3. c. Partnership;
      4. d. Proprietorship; or
      5. e. Other;
   4. (4) Contact information including name and e-mail address;
   5. (5) Address of the location where records will be available for audit;

   6. (6) The type of product and the estimated gallons per year of the product to be imported, as follows;

      1. a. Automotive gasoline;
      2. b. Aviation gasoline;
      3. c. Diesel fuel;
      4. d. Jet fuel;
      5. e. Heating oil; and
      6. f. Other;
   7. (7) Address and capacity size of each bulk storage facility;
   8. (8) The type of product to be stored at each facility;
   9. (9) Whether the storage facilities are owned or leased;
   10. (10) The signature and title of person completing the form; and
   11. (11) The name of the applicant for whom the form is completed.
