1. (a) Upon approval of a mooring permit application, the division shall issue a mooring permit to the applicant.

2. (b) The permit shall include:

   1. (1) The address of the shorefront property where the mooring(s) are located;
   2. (2) The date the mooring permit was approved;
   3. (3) A statement that the mooring permit shall not be transferred to any other person or location by gift, sale, lease or rent; and

   4. (4) Any conditions attached to the mooring permit, such as but not limited to:

      1. a. Whether orange buoys with non-strobe flashing orange lights shall be required to establish the perimeter of the mooring field;
      2. b. The maximum number of moorings allowed at the specified shorefront property;
      3. c. That boats be required to be tied bow and stern;
      4. d. The removal of a line extending from the dock to the mooring;
      5. e. A final court ruling, if litigation occurred regarding the mooring permit;
      6. f. The specific location requirements of the placement of the mooring(s); and
      7. g. The maximum boat size that may be attached to the mooring.
