Any person who utilizes the toll-free hotline to report drug activity shall be asked for the following information:

1. (a) Whether the activity is taking place within a school zone, and if so, the name of the school zone;
2. (b) Whether the caller wishes to remain anonymous, and if not, the name, address, and telephone number of the caller;
3. (c) Name, address, and telephone number of the individual who may be possessing, manufacturing, selling, dispensing, or possessing with an intent to sell, any controlled drug or analog, if known;
4. (d) Physical description of the individual who may be possessing, manufacturing, selling, dispensing, or possessing with an intent to sell, any controlled drug or analog; and
5. (e) Any other information the caller may wish to report.
