Any agency or approved manufacturer that pays the renewal fee after the annual anniversary date of approval shall be charged as follows:

1. (a) If 30-90 days after the annual anniversary date of approval, a $100.00 fee in addition to the annual renewal fee; or
2. (b) If 91-180 days after the annual anniversary date of approval, a $500.00 fee in addition to the annual renewal fee.
