1. (a) Records shall be kept by the manufacturer and the agency for a minimum of 8 years.

2. (b) The records required in (a) above, shall include:

   1. (1) The number of state labels applied to modular buildings and building components;
   2. (2) Which labels, based upon serial number, have been applied to which modular buildings or building components and the destination;
   3. (3) Disposition of any damaged or rejected labels; and
   4. (4) The location and custody of all unused labels.
3. (c) The manufacturer shall complete and submit a state label report on form DSFM 119, “Manufacturer Monthly Label Report”, revised 2/2024, to the department on a monthly basis.
4. (d) The third party agent shall complete and submit a state label report on form DSFM 118, “Third Party Inspection Agency Monthly Label Report”, revised 2/2024, to the department on a monthly basis.
5. (e) In addition to (c) above, the manufacturer shall complete form DSFM 56, “Modular Building Label Record” revised 2/2024, and submit it to the department within 5 business days of shipment of NH labeled modular units.

6. (f) DSFM 56, “Modular Building Label Record” revised 2/2024, shall be submitted to the department at the following address:
