Issuance of State Labels

- (a) Labels shall be issued by the department.
- (b) Agencies shall order labels from the department by completing, signing, and submitting the “Request & Payment for New Hampshire Labels” form DSFM-115, revised 2/2024.
- (c) Labels shall be sent to the agency, return receipt requested.
- (d) Upon issuing a label, the agency shall forward the label fee, paid for by the manufacturer in accordance with Saf-C 3309.11, to the department.
