1. (a) The permanent manufacturer's data plate located in the vicinity of the state label shall include the following information:

   1. (1) Name and address of manufacturer;
   2. (2) Manufacturer's serial number;
   3. (3) Manufacturer's plan approval designation-model number and name;
   4. (4) Construction classification;
   5. (5) Use and occupancy;
   6. (6) Seismic zone;
   7. (7) Wind velocity load;
   8. (8) Roof live load, including ground snow load;
   9. (9) Floor live load;
   10. (10) Fire rating for exterior walls, if applicable;
   11. (11) Thermal transmittance values;
   12. (12) Date of manufacture;
   13. (13) Destination;
   14. (14) State label numbers; and
   15. (15) Agency label numbers.
2. (b) If the shape or size of a modular building or building component is such that the data plate cannot be permanently attached, the information shall be placed in a manual crated with the component or on a tag attached to the crate in which the component is shipped. The manual or tag shall be presented to the owner upon transfer of possession.
