On an annual basis, no later than 45 days prior to the expiration of agency approval pursuant to Saf-C 3304.06, the third party shall submit the following to the department:

1. (a) A completed application and supporting documentation, pursuant to Saf-C 3304.02; and
2. (b) A renewal fee of $1,000.00 to the department, pursuant to RSA 205-C:6.
