Ordering Stickers and On-Demand Sticker Backings

- (a) All stickers shall be ordered, in writing, on form RDMV 967 (rev. 11/14), or similar form. Electronic requests shall be accepted as long as the request contains the signature of a person authorized to order stickers.
(b) The written request pursuant to (a) above shall include the following:
- (1) Inspection station number;
(2) Type of sticker requested:
- a. Motor vehicle stickers and year;
- b. Motorcycle stickers and year; or
- c. Monthly insert decals;
- (3) Name and address of business;
- (4) Number of stickers; and
- (5) Signature of person authorized pursuant to RDMV 967 to order stickers.
- (c) Each inspection station shall allow a period of 10 days for requests to be processed.
- (d) On-demand sticker backings shall be ordered directly through the supplier in accordance with contractual agreements between the supplier and inspection stations.
