- (a) Each approved drivers’ school shall complete and submit in a typed format, form DSMV 99, “Driver Education Enrollee Report” (rev 09/17) to the division, prior to the third meeting of each class.
- (b) The person in charge of an approved drivers’ school shall sign and date the form under the penalty of unsworn falsification.
