1. (a) Upon receipt of an abstract of conviction of an offense set forth in Saf-C 3003.01(b) - (e) or a habitual offender certification order, the supervisor of financial responsibility shall issue a notice to the person whose license is under suspension or revocation.

2. (b) The notice shall include the following information:

   1. (1) A statement that all registrations issued in the person's name shall be suspended in 30 days;

   2. (2) A statement that all registrations and plates shall be forwarded to the department at the

      conclusion of the 30-day period, unless a hardship registration has been obtained within the 30-day period;

   3. (3) The procedure for obtaining a hardship registration; and
   4. (4) A statement that the person shall be required to present the registration suspension notice to the department, if a hardship registration shall be requested, in order to prove that the person is qualified, pursuant to RSA 261:180 III, to obtain a hardship registration.
