1. (a) Each applicant for a local permit to sell permissible fireworks shall obtain a permit application from each municipality in which the person intends to sell or market permissible fireworks.

2. (b) Each applicant for a local permit to sell permissible fireworks shall submit the following on the permit application:

   1. (1) Seller’s name, address and date of birth;
   2. (2) Business name and address;
   3. (3) That only permissible fireworks will be sold or marketed; and
   4. (4) Location(s), address(es) and physical description of all storage areas, including a site plan.
