Application for a Local Permit to Sell Display and Consumer Fireworks

- (a) Each applicant for a local permit to sell display and consumer fireworks shall obtain a permit application from the municipality.
(b) Each applicant for a local permit to sell display and consumer fireworks shall submit the following on the permit application:
- (1) Seller’s name, address and date of birth;
- (2) Business name and address;
- (3) Type of fireworks that will be sold; and
- (4) Location(s), address(es) and physical description of all storage areas, including a site plan.
