1. (a) Each vessel agent shall mail a weekly report to the department and mail a copy to the tax collector of the municipality where the vessel agent is located.

2. (b) Each vessel agent shall furnish the following on the weekly report:

   1. (1) Name of the city or town where the vessel agent is located;
   2. (2) Period the report begins and ends;
   3. (3) Number of vessels registered;
   4. (4) Amount of tax collected; and
   5. (5) Vessel agent’s signature.

3. (c) The weekly report shall be submitted along with:

   1. (1) The department's copy of all registrations; and
   2. (2) All fees due to the department in the form of a check or money order.
