Replacement Decals

- (a) In the event an original decal is lost, stolen, or damaged, the owner shall complete Form RDMV 415 “Application for Replacement Boat Decals” (rev. 07/25) and submit it to the department.
- (b) The owner shall sign form RDM 415 and the owner’s signature shall certify that the replacement boat decal is needed for the reason indicated on the application.
- (c) Upon receipt of a completed form RDMV 415 and valid vessel registration certificate, and payment of the required fee of $3.00, the department shall issue a new decal and new registration certificate reflecting the new decal number.
