The department shall furnish application forms for vessel registrations at the following locations:

1. (a) Division of Motor Vehicles

   Stephen E. Merrill Building

   23 Hazen Drive

   Concord, N.H. 03305;

2. (b) Division of State Police

   Bureau of Marine Patrol

   31 Dock Road

   Gilford, N.H. 03249;

3. (c) Any substation; and
4. (d) Any vessel agent’s place of business.
