Each request for a hearing on a suspended or revoked license shall:
- (a) Be in writing from the person requesting the hearing; and
(b) Shall contain the following information:
- (1) Name;
- (2) Address;
- (3) Telephone number; and
- (4) The reason(s) for contesting the suspension or revocation.
