1. (a) The report of investigation of accidents required pursuant to RSA 264:26, shall be submitted to the division on form DSMV-159.

2. (b) The investigating law enforcement officer shall submit the following on form DSMV-159:

   1. (1) The location of accident, including the:

      1. a. Route number/street name;
      2. b. Distance from and name of nearest intersecting road, bridge, town line;
      3. c. City or town, and county;
      4. d. Relative distance to nearest nodes or mile markers, if interstate highway; and
      5. e. Posted speed;
   2. (2) The day, date and time of accident;

   3. (3) The total number of:

      1. a. Killed;
      2. b. Injured; and
      3. c. Vehicles;

   4. (4) The time that the:

      1. a. Police were notified;
      2. b. Police arrived; and
      3. c. Ambulance arrived;
   5. (5) Which police department was involved;

   6. (6) The identification of drivers, pedestrians, and bicyclists, including:

      1. a. Their names;
      2. b. Their addresses;
      3. c. Their license numbers;
      4. d. Their license classifications;
      5. e. Their dates of birth;
      6. f. Their sex; and
      7. g. An indication as to whether license restrictions/endorsements were complied with;

   7. (7) Identification of owners and vehicles involved, including:

      1. a. Their names;
      2. b. Their addresses;
      3. c. Their vehicle identification numbers;
      4. d. The make of each vehicle;
      5. e. The year of manufacture for each vehicle;
      6. f. The plate numbers and state where issued;

      7. g. The trailer plate number and state where issued, if

         applicable; and

      8. h. The insurance company and policy number or indication as to whether an insurance card, DSMV 385, was issued;

   8. (8) An indication as to whether the accident involved:

      1. a. A commercial vehicle; or
      2. b. Hazardous materials;
   9. (9) The probable points of impact for any vehicle;
   10. (10) A diagram of accident;
   11. (11) A recommendation to division relative to hearing or driver re-examination, including a report describing the reason for the recommendation;
   12. (12) An indication as to whether vehicle was towed, and if so, by whom and to what destination;
   13. (13) A narrative description of accident;
   14. (14) Summaries of any driver's statements;
   15. (15) The name, address, and phone number of each occupant or witness;
   16. (16) The signature of the investigator, and the date, department, division, or troop; and
   17. (17) An indication as to whether photographs of the scene were taken.
