1. (a) An SR-22 certificate filed pursuant to any law or rule shall include:

   1. (1) The insured's name and date of birth;
   2. (2) The insured's address;
   3. (3) The name of the insurance company;
   4. (4) The policy number;
   5. (5) The effective date of the policy; and
   6. (6) The type of coverage in effect.

2. (b) An SR-26 certificate filed pursuant to any law or rule shall include:

   1. (1) The insured's name and date of birth;
   2. (2) The insured's address;
   3. (3) The name of the insurance company;
   4. (4) The policy number;
   5. (5) The effective date of the policy; and
   6. (6) The effective date of the cancellation or termination of the policy.
