1. (a) The manufacturer's certificate of origin shall be the official form representing proof of ownership for new vehicles delivered in this state.

2. (b) A manufacturer's certificate of origin shall include the following:

   1. (1) Manufacturer's name;
   2. (2) Vehicle identification number;
   3. (3) Vehicle's year of manufacture, make, and model;
   4. (4) Number of cylinders; and
   5. (5) General description of body style, if any.
