Change of Name

(a) In the event a person changes his or her name, he or she shall furnish the following on form DSMV 30 “Record Change Request” (rev. 3/22):
- (1) Former name;
- (2) New name;
- (3) Date of birth; and
- (4) Signature.
(b) As evidence of a new name, a person shall present a copy of one of the following:
- (1) Name change petition from the court of probate;
- (2) Marriage certificate;
- (3) Divorce decree;
- (4) Passport;
- (5) Adoption decree; or
- (6) Court decree.
- (c) In the event a corporation, partnership, or other legal entity changes its name, it shall be presumed that the change constitutes a transfer of ownership, unless satisfactory proof to the contrary is furnished, such as a certificate from the secretary of state's office showing the name was changed and that no corporate officers have changed.
