1. (a) Pursuant to RSA 263:97, I(a), the clerk of court having jurisdiction shall notify the department of the conviction of any resident or non-resident commercial driver.

2. (b) For the purposes of (a) above, the clerk of court shall notify the department of the following:

   1. (1) Docket number;
   2. (2) Name of court;
   3. (3) Driver's legal name and permanent address;
   4. (4) Date of violation;
   5. (5) Motor vehicle registration;
   6. (6) Description of vehicle;
   7. (7) Violation(s) committed;
   8. (8) RSA violated;
   9. (9) Name of complainant;
   10. (10) Plea;
   11. (11) Finding; and
   12. (12) Sentence imposed.
