Jumper Registration

(a) All bungee jumpers shall supply the following information upon registering:
1. (1) Name;
2. (2) Address;
3. (3) Telephone number;
4. (4) Medical conditions;
5. (5) Age; and
6. (6) Written parental consent, if the jumper is age 16 or 17.
