1. (a) In any case in which a suspension of operation is ordered by the commissioner or his authorized agent pursuant to RSA 321-A:7, the written notice required by that statute shall be issued on the "Notice of Suspension" form.

2. (b) The form shall include all of the following:

   1. (1) The owner or operator's name;
   2. (2) Date of issuance;
   3. (3) Order to cease operation;
   4. (4) Identification of carnival or amusement rides covered by such order;
   5. (5) Notification that the carnival or amusement ride shall remain closed until permission to reopen is expressly granted by the commissioner or his agent;
   6. (6) Reason for suspension;
   7. (7) Required corrective action; and

   8. (8) Affidavit of service which shall include:

      1. a. Certification that the order was served on the owner or operator;
      2. b. Date and time of such service;
      3. c. Signature of person making service.
