Physical Examination

- (a) All applicants for a school bus certificate shall provide their employer with a copy of the applicant’s valid USDOT medical certificate, if a commercial driver license holder, or a completed current physical examination report Form DSMV 649, “School Bus Driver Physical Exam”, (rev. 06/24) signed by a medical examiner, if a non-commercial driver license holder. For the purposes of this section, “current” means that the physical examination has been completed no later than 90 days prior to the date of application.
(b) The physical examination shall be completed during the following periods:
- (1) Prior to the issuance of an initial school bus certificate, pursuant to Saf-C 1304.01(a)(5); and
- (2) Every 2 years following the issuance of a school bus certificate.
- (c) The medical examiner must sign and date the form, and in doing so, shall certify “I hereby certify that I am familiar with the present medical condition of the above named individual, who is an applicant for a New Hampshire School Bus Certificate. I further certify that said applicant is physically capable of operating a school bus. I understand that this certification and recommendation will be used in determining the applicant’s eligibility to receive a School Bus Driver Certificate.”
