1. (a) An entity that requests approval of its training program shall submit the following to the department:

   1. (1) A short statement of the certification for which the entity wants its training program to be evaluated, including the educational requirements set forth in Saf-C 1204;
   2. (2) The training program’s curriculum;
   3. (3) A copy of the training program’s educational materials;

   4. (4) A statement demonstrating:

      1. a. The integrity and proctoring of exams; and
      2. b. The educational subject matter incorporated in the exams; and
   5. (5) A copy of a sample certificate of each certificate for which the entity is seeking approval.
2. (b) Upon receipt of a request for approval of its training program, the state fire marshal shall schedule a time for the entity to provide a presentation to the commissioner or his designee.
