- (a) The fee for a new application for a firm certificate of registration shall be $100.00. This fee shall also apply to each new application for branch locations.
- (b) The fee for a firm making application for multiple disciplines shall be $50.00 for each additional discipline.
- (c) The fee for a new application for an individual certificate shall be $50.00.
- (d) The fee for an individual making application for multiple disciplines shall be $5.00 for each additional discipline.
- (e) The fee for a renewal application for a firm certificate of registration shall be $90.00. This fee shall also apply to each renewal application for branch locations.
- (f) The fee for a renewal application for an individual certificate shall be $40.00.
