Required Notifications

Effective Nov 6, 2010(See Revision Note at chapter heading for Saf-C 1200) #9810-A, eff 11-6-10Commissioner, Department of Safety

(a) A holder of an individual certificate shall notify the department, in writing, by mail, to the address set forth in Saf-C 1204.03(a), within 30 calendar days of the following:
1. (1) A change in name or address; or
2. (2) A change in employer.
