1. (a) Each individual certificate issued shall contain the following:

   1. (1) Name and address of individual;
   2. (2) Certificate number;
   3. (3) Any endorsements;
   4. (4) Date of expiration; and
   5. (5) Signature of the state fire marshal.
2. (b) An individual certificate issued shall expire on the last day of the individual’s birth month, 3 years from the date of issuance.
