The department shall issue a limited privilege license pursuant to RSA 263:57-b upon receipt of all of the following:

1. (a) A completed form DSMV 450 “Application for Driver License or Non-Driver ID Card,” (rev. 06/23);
2. (b) Certified order from a New Hampshire court, granting a petition for a limited privilege license, bearing the court’s certification on every page, including all pages of documents submitted to the court by the applicant and incorporated by reference in the order;
3. (c) Proof of financial responsibility in accordance with RSA 265-A:28;
4. (d) Certificate of installation of an approved enhanced technology ignition interlock device; and
5. (e) Payment of the fee required by RSA 263:42, V(b).
