1. (a) The director shall issue at no charge a vanpool operator's permit to any licensed driver who transports no fewer than 8 nor more than 15 people to and from work on a non-profit basis, when the vehicle used is of a type specified in RSA 376:2, XII.

2. (b) Each applicant for a vanpool operator’s permit shall submit a letter to the:

   Division of Motor Vehicles

   Attn: Driver Licensing

   23 Hazen Drive

   Concord, N.H. 03305.

3. (c) Each letter pursuant to (b) above shall include the following:

   1. (1) Name and date of birth;
   2. (2) Legal address;
   3. (3) Height and weight;
   4. (4) Gender;
   5. (5) Color of hair and eyes;
   6. (6) Driver license number;

   7. (7) The following certified statements:

      1. a. The applicant has paid all residence taxes for which they are liable;
      2. b. The applicant has filed an insurance certificate with the director;
      3. c. The applicant has no mental or physical handicap which would be detrimental or incapacitate the applicant from holding a vanpool operator's permit;
      4. d. The applicant's driver license is not under suspension or revocation in any jurisdiction, and no legal or administrative action relating to the applicant's operation of a motor vehicle is pending against them in any jurisdiction; and
      5. e. The applicant is aware of their responsibility to notify the director of any change in the status of an insurance certificate filed with the director in connection with a vanpool operator's permit; and
   8. (8) Signature of the applicant and date the letter was signed.
4. (d) Each applicant shall submit the information in (c) above to the division, along with a certificate from an insurance company indicating that the vehicle used for vanpooling service is covered by a liability insurance policy.

5. (e) The insurance policy shall be for at least the following amounts:

   1. (1) For injury to or death of one person, $100,000;
   2. (2) For injury to or death of more than one person, $300,000; and
   3. (3) For damage to the property of others, $20,000.
6. (f) Each applicant for a vanpool operator's permit shall produce a valid New Hampshire driver license of the proper classification for the vehicle used for vanpooling service.

7. (g) Each applicant shall satisfactorily complete the following in the order reflected:

   1. (1) The visual acuity examination set forth in Saf-C 1004.01; and
   2. (2) The road skills examination set forth in Saf-C 1006.01, in the vehicle that shall be used for vanpooling service.
8. (h) Each vanpool operator's permit shall be in the form of a sticker affixed to the applicant's New Hampshire driver license.
