1. (a) A business enterprise seeking the issuance of a certification statement of good standing from the department shall, in addition to providing the information in Rev 2409.01 (a) and (c), submit to the department:

   1. (1) Returns due but have not yet been filed with the department; and
   2. (2) A copy of the federal tax return, as filed with the IRS.
