1. (a) “Employer enrollment oversight” means a period of time when a member met the eligibility requirements for membership but was not properly enrolled in the system on time.
2. (b) A member seeking to acquire additional service credit under the enrollment oversight provisions shall complete and file NHRS Form 10.

3. (c) The member shall complete Part I of NHRS Form 10 by providing the following:

   1. (1) The member’s name;
   2. (2) The dates of the disputed service;
   3. (3) The employer’s name;
   4. (4) The member’s Social Security number;
   5. (5) The member’s signature;
   6. (6) The date of the application;
   7. (7) The member’s telephone number; and
   8. (8) The member’s address.

4. (d) The employer identified in Part I, through its certifying official, shall complete Part II of NHRS Form 10 by providing the following:

   1. (1) The name of the certifying official;
   2. (2) A determination of eligibility;
   3. (3) The name of the employer;
   4. (4) The dates of the dispute service;
   5. (5) The employer’s agreement or disagreement with the claim;
   6. (6) The date of completion;
   7. (7) The signature of the employer’s authorized agent;
   8. (8) The employer’s telephone number;
   9. (9) The employer’s mailing address; and
   10. (10) The certifying official’s title.

5. (e) The member’s current employer, through its certifying official shall complete Part II of NHRS Form 10 by providing the following:

   1. (1) The employer’s name;
   2. (2) The certifying official’s name;
   3. (3) The member’s current annual base salary;
   4. (4) The date of completion;
   5. (5) The employer’s telephone number;
   6. (6) The employer’s mailing address;
   7. (7) The certifying official’s title; and
   8. (8) The certifying official’s signature.
6. (f) Any member who claims financial hardship and the inability to pay his or her proportional share of a purchase of creditable service under Ret 308.12 shall file as a financial statement, Form FS-1.
7. (g) The employer thru its certifying official by signing Part II of NHRS Form 10 shall certify the accuracy of the information provided.
