1. (a) The business applying for funds shall provide on the statement of eligibility the following information:

   1. (1) The business name, address, website and telephone number;
   2. (2) The names of the chief executive officer and all other officers; and
   3. (3) The contact person's name, business address, telephone number and email address.
2. (b) All information shall be legible.

3. (c) The business shall include a copy of the following:

   1. (1) A lease, purchase and sales agreement, or deed for real estate, if intending to locate in New Hampshire; and
   2. (2) A certificate showing good standing with the New Hampshire secretary of state.

4. (d) The applicant shall agree to:

   1. (1) Provide a training evaluation report; and
   2. (2) Abide by all non-discrimination laws.

5. (e) The contact person representing the business shall:

   1. (1) Sign and date the form; and

   2. (2) Certify by his or her signature that:

      1. a. The employees receiving training are residents of or are employed in New Hampshire;
      2. b. The business is in compliance with all applicable federal, state, or local laws and regulations; and
      3. c. A training evaluation report due on any previously funded program has been completed and submitted.
