General Requirements for Written Communications

All written communications submitted to the commission shall:
- (a) Be dated;
- (b) Be signed. An electronic signature or “/s/” mark, with the full name of the signing person provided with the electronic signature or “/s/” mark, shall be an acceptable means of signing the filing;
- (c) Identify the name and mailing address, email address, and telephone number of the person submitting the document and the party or parties on whose behalf it is submitted, if any;
- (d) Identify the appropriate docket number, order number, or other pertinent subject matter;
- (e) Be typed on 8-1/2 by 11 inches sized paper;
- (f) Have no less than one-inch margins on all sides;
- (g) Contain sequentially numbered pages;
- (h) If a pleading or testimony, be double-spaced;
- (i) If a pleading, be filed as a separate document from any supporting materials, such as testimony and supporting attachments;
- (j) If materials supporting a pleading, be organized with a single series of numerical page designations, for example Bates numbering; and
- (k) If supporting a pleading that is over 20 pages, include a table of contents.
