2. (2) Date;
3. (3) Account number and title;
4. (4) Estimated life - present and proposed in years;
5. (5) Net salvage - present and proposed by percentage;
6. (6) Depreciation rate -present and proposed by percentage;
7. (7) Net annual change in dollars; and

8. (8) Justification and supportive documentation for the proposed change in depreciation rates.
