1. (a) Awards shall be made in the following amounts:

   1. (1) Year 1: 10% or $10,000, whichever is less;
   2. (2) Year 2: 10% or $10,000, whichever is less;
   3. (3) Year 3: 20% or $20,000, whichever is less;
   4. (4) Year 4: 30% or $30,000, whichever is less; and
   5. (5) Year 5: 30% or $30,000, whichever is less.
2. (b) No recipient shall receive more than $100,000 in repayments.
3. (c) The applicant shall complete an alternate W-9 form to permit payment processing.
