(a) Each applicant for tuition repayment under this program shall make application on Form PEC-CART, application for CART/sign language interpreter tuition repayment, by supplying the following information:
- (1) Applicant’s first name and last name;
- (2) Applicant’s current address;
- (3) Applicant’s current employer, or if self-employed, applicant’s EIN number;
- (4) Employer’s address, if applicable;
- (5) Beginning and ending date of service for which an award is sought;
- (6) Name and title of employer’s certifying official; and
- (7) Signature of employer’s certifying official.
- (b) The application shall be signed and dated by the applicant.
- (c) The applicant shall attach copies of bills, or receipts, detailing the tuition paid for training as a CART provider or sign language interpreter for which the applicant is seeking repayment.
- (d) The applicant shall provide evidence of certification/licensure as specified in Pos 1501.03.
