- (a) The applicant for renewal shall complete and submit the "Universal Application for License Renewal", required by Plc 308.05, by providing the information described in Plc 308.06, and the additional required information and documentation specified in Plc 308.07.
- (b) The applicant shall sign and date the application as required by Plc 308.08.
