The effect of the signature required by Plu 405.03(b)(4) shall be:

1. (a) The signer's acknowledgement that, under RSA 641:2, I, knowingly making a false statement on the application form is a misdemeanor; and
2. (b) The signer's certification that the information provided on the renewal application form is complete and accurate to the best of the signer's knowledge and belief.
