1. (a) On the form to be used by plumbing businesses applying for licensure pursuant to RSA 329-A:10, II, the applicant shall identify the business as a corporation, a limited liability company or a partnership.

2. (b) The applicant shall also provide:

   1. (1) The date the applicant submits the form;
   2. (2) The name of the corporation, limited liability company or partnership;
   3. (3) The full mailing address of the corporation, limited liability company or partnership;
   4. (4) The full physical address of the corporation, limited liability company or partnership;
   5. (5) The telephone number of the corporation, limited liability company or partnership;
   6. (6) The name of a corporate officer, managing member of the limited liability company or partner, as applicable, holding a currently valid New Hampshire master plumber license;
   7. (7) The number of that person’s master plumber license;

   8. (8) As applicable, one of the following titles for that person:

      1. a. President of the applicant corporation;
      2. b. Vice president of the applicant corporation;
      3. c. Treasurer of the applicant corporation;
      4. d. Managing member of the applicant limited liability company; or
      5. e. Partner in the applicant partnership; and
   9. (9) If the business is a corporation, whether that person is the sole shareholder.

3. (c) The corporate officer, managing member of a limited liability company or partner named pursuant to (b)(6) above shall place the following beneath the preprinted promise that the applicant will perform its work in accordance with the state plumbing code or any more stringent local code:

   1. (1) His or her printed name;
   2. (2) His or her signature; and
   3. (3) The date of signing.
