In addition to the documentation required by Plc 304.04, the applicant for an initial license to practice massage therapy shall provide the following documentation:
- (a) An official copy of the diploma or certificate received from the qualifying massage therapy school;
- (b) Documentation of passing the written exam required by Plc 904.05 and, if not the MBLEx, proof that the examination meets the criterion in Plc 904.05(b);
- (c) As required by RSA 328-B:6, I(d), a statement verifying that the applicant is in good mental and physical health, meaning physically and mentally capable of performing massage therapy, or an explanation of the circumstances that prevent the applicant from so verifying; and
- (d) As required by RSA 328-B:6, I(c), a statement that the applicant has not been convicted of any crime involving violence inflicted on a person or threatened against a person, or any sexually-related crime, in New Hampshire or any other jurisdiction, or an explanation of each conviction including the jurisdiction, date, crime, sentence imposed, and any rehabilitative action(s) the applicant has taken subsequent to being charged with the crime.
