1. (a) Each renewal license issued under this chapter shall specify:

   1. (1) The name of the licensee;
   2. (2) The profession in which the licensee is authorized to practice;
   3. (3) The effective date of the license;
   4. (4) The license number; and
   5. (5) Any conditions imposed pursuant to Plc 311.06.

2. (b) A renewal license shall be effective for 2 years from the date of issuance unless:

   1. (1) A complete renewal application is filed on or before the expiration date; or
   2. (2) The license is sooner suspended or revoked pursuant to Plc 311 or other applicable law.
