An individual who is not already certified as a CHW in New Hampshire shall apply for initial certification by complying with Plc 304.01, subject to the following:
- (a) On or with the profession-specific supplement required by Plc 304.01(a)(3), the applicant shall provide the information and documentation specified in Plc 2003.05;
- (b) No criminal background check shall be required; and
- (c) The fee shall be the application processing and licensing fee specified in Plc 1002.54, provided that an applicant who is active military or a military spouse shall not pay the fee.
