1. (a) A licensee may claim credit towards the continuing competence requirement for completing self-directed education as defined in Plc 1302, provided that the course sponsor or provider issues the documentation described in (c), below.
2. (b) The licensee may claim the hours attributed by the course sponsor or provider.

3. (c) Documentation of self-directed education shall be:

   1. (1) An official transcript showing successful completion of the course, the date of the course, and the credits earned; or

   2. (2) The official certificate of completion issued to the licensee by the course sponsor or provider showing:

      1. a. The licensee’s name;
      2. b. The name of the course;
      3. c. The name of the provider or sponsor;
      4. d. The beginning and ending date(s) of the course, if applicable;
      5. e. The number of contact hours credited by the provider or sponsor; and
      6. f. The printed name and signature or stamp of the instructor or a representative of the provider or sponsor.
