1. (a) Each renewal license to practice court reporting issued by the OPLC shall specify:

   1. (1) The name of the licensee;
   2. (2) The effective dates of the license;
   3. (3) The license number; and
   4. (4) The method the licensee shall be authorized to use.

2. (b) Renewal licenses shall:

   1. (1) Be valid for 2 years from the date of issuance, provided that the timely filing of a complete renewal application shall continue the validity of the license being renewed until final action is taken on the renewal application;
   2. (2) Expire 2 years from the date on which the license was issued unless a timely and complete renewal application is filed or the license is sooner suspended or revoked pursuant to Plc 1208; and
   3. (3) Be subject to renewal biennially.
