1. (a) Subject to (g) - (j), below, the fee for any inspection related to initial licensure shall be $150.
2. (b) Subject to (g) - (j), below, the fee for any reinspection related to initial licensure shall be $100.
3. (c) Subject to (g) - (j), below, the fee for any inspection related to renewal licensure shall be $150.
4. (d) Subject to (g) - (j), below, the fee for any reinspection related to renewal licensure shall be $100.
5. (e) Subject to (g) - (j), below, the fee for any inspection related to reinstatement of licensure shall be $150.
6. (f) Subject to (g) - (j), below, the fee for any reinspection related to reinstatement of licensure shall be $100.

7. (g) For general anesthesia, deep sedation, and moderate sedation permits for dentistry:

   1. (1) The fee for an initial facility inspection under Den 304.05 shall be $300; and
   2. (2) The fee for a comprehensive evaluation under Den 304.05 associated with initial licensing shall be $750.
8. (h) The fee for inspection of an electrology school for initial licensure shall be $150.
9. (i) The fee for an initial inspection of a funeral home, funeral chapel, or crematory for initial licensure shall be $250.
10. (j) The fee for an initial site visit to a nursing program for initial licensure shall be $100.
